The websites of the Andalusian Healthcare Quality Agency are adapted to the requirements of the  “Ley Orgánica 15/1999, de 13 de diciembre, de Protección de Datos de Carácter Personal (LOPD)”, and to the “Real Decreto 1720/2007, de 21 de diciembre”, known as the “Reglamento de desarrollo de la LOPD”. It also complies with “Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation)”, as well as the “Ley 34/2002, de 1 de julio, de Servicios de Sociedad de la Información y Comercio Electrónico (LSSICE o LSSI)”. In the cases in which to access to some of the pages or services of the Andalusian Healthcare Quality Agency it is necessary the collection of personal data through e-mail or form, only those data that are strictly necessary to be able to attend your consultations, to make possible your participation in some project or to inform you properly about some service given by the ACSA will be requested. We inform you that these data will be treated in accordance with the established in the current regulations related to the Protection of Personal Data and other rules of development. In compliance with the articles 13 and 14 of the General Data Protection Regulation, we inform you the following: a) The personal data that you provide us will be used to attend the requests of consultation sent, as well as to be able to inform you about the services offered from the different areas of activity of the Andalusian Healthcare Quality Agency (certification of centers and units, of sanitary professionals, of continuous formation, of web pages and blogs, distinctive Appsaludable, safety of the patient, professional development, etc.) or to facilitate your participation in some project of the ACSA, remaining stored during the necessary time to fulfill the stipulated legal obligations. Your data will not be transferred to third parties, unless you have a legal obligation. The legal base of this treatment is based on the consent that you give us when filling in and sending the form or e-mail, without which we could not comply with the described purpose. b) As the owner, the entity responsible for this treatment is the Fundación Pública Andaluza Progreso y Salud, which supports theAndalusian Healthcare Quality Agency, whose address is Av. Exercise of personal data rights
d) You can exercise your rights of access, rectification, suppression, portability of your data, and the limitation or opposition to its treatment, by requesting it in writing, with a copy of your ID, to the Fundación Pública Andaluza Progreso y Salud in Avda. Accuracy and veracity of the data
The user guarantees that the personal data provided through the different forms are true, being obliged to communicate any modification of them. In addition, the user is obliged to keep his or her data updated at all times, and is solely responsible for the inaccuracy or falsity of the data provided and for any damage that may be caused to the Andalusian Healthcare Quality Agency as the owner of this website.
