You can also collect your application in the Provincial Offices of the
Regional Ministry for Equality and Social Welfare in your Town Council, or you can
download it in electronic format (you need
Acrobat Reader
) to print it and send it to the following address:

Do not forget to stick a
recent card-size photograph and to attach an
ID card photocopy to your application.
