Tras una carrera brillante decidi\u00f3 inclinarse por la cirug\u00eda cardiovascular y se form\u00f3 en los servicios de las universidades de Birmingham, Stanford, Leiden y el Instituto de Cardiolog\u00eda de Montreal. Una ocasional visita a Sevilla y el encuentro con un grupo de cardi\u00f3logos formados en la cardiolog\u00eda moderna le descubri\u00f3 las posibilidades de realizar cirug\u00eda cardiovascular en la capital andaluza. Consigui\u00f3 permiso \u2013no remunerado\u2013 para trabajar en la Diputaci\u00f3n y, poco despu\u00e9s, en vista de los resultados obtenidos, este organismo cre\u00f3 el servicio correspondiente cuya oposici\u00f3n gan\u00f3. Sin embargo, por razones pol\u00edticas de la \u00e9poca (a las que no fue ajeno el Dr. Mart\u00ednez Bordi\u00fa, yerno de Franco), la Diputaci\u00f3n decidi\u00f3 suspender la creaci\u00f3n del servicio y enviar los enfermos a Madrid.<\/p>\n<p>Rivera sigui\u00f3 operando en el Hospital de las Cinco Llagas (hoy Parlamento de Andaluc\u00eda), donde practic\u00f3 intervenciones pioneras en Espa\u00f1a. Destaca fundamentalmente el d\u00eda 5 de octubre de 1970, en que llev\u00f3 a cabo la primera anastomosis aorto-coronaria con puente o \u2018by-pass\u2019 de vena safena. Cuando el caso se present\u00f3 en el Congreso de la Asociaci\u00f3n de Cardi\u00f3logos del Sur en Ja\u00e9n (1971), ya llevaba 24 sin mortalidad.<\/p>\n<p>El deterioro del edificio de Las Cinco Llagas oblig\u00f3 a su abandono, coincidiendo con la convocatoria en Madrid de la jefatura de un nuevo servicio de su especialidad en el reci\u00e9n construido Hospital de la Diputaci\u00f3n (actual Gregorio Mara\u00f1\u00f3n). El doctor Rivera compiti\u00f3 y gan\u00f3 la oposici\u00f3n y, con su familia (cinco hijos sevillanos), march\u00f3 a Madrid, donde sigui\u00f3 ejerciendo su profesi\u00f3n.<\/p>\n<p>Rivera fue cofundador de las sociedades de Cirug\u00eda Cardiovascular y de la European Association of Cardiothoracic Surgery, que presidi\u00f3 en 1992. Fue presidente del Consejo General de Colegios M\u00e9dicos de Espa\u00f1a (1982-1986). En 2014 le fue concedida la primera Gran Cruz de la Sanidad Madrile\u00f1a.
